The Grampians Regional Palliative Care Team works closely with the Palliative Care Service Providers across the region to assist in the ongoing development and delivery of effective community based palliative care services. This includes elements of education, collaborative strategic planning, preparation of written materials, policies and procedures, quality improvement processes, and consultation. The Grampians Regional Palliative Care Team is committed to providing a variety of quality education to a broad range of health professionals who are striving for better palliative care practice.
